چکیده

Objectives: The aim of this study was to demonstrate that measuring the medial gap before bone resection during total knee arthroplasty (TKA) provides an optimum adjustment in varus knees. Methods: Patients were separated into two groups, Group 1 being those whose joint measured prior and 2 comprising who underwent conventional technique without measuring. with a custom-made device up point corrected aligned along its mechanical axis. Medial distances, distal femoral cut thicknesses, amounts tibial calculated, internal distances after cutting, thicknesses trial inserts recorded. A comparison made between groups terms number patients requiring additional distribution insert thicknesses. Results: Extra resections performed (5.7%) 10 (28.6%) 2. In 1, where measured, need for statistically less. (p=0.018). size by group, on whom 8 mm had been used significantly greater (p=0.024). Conclusion: Measuring may prevent repeated recutting resections. Furthermore, we can use method avoid disadvantages technique.

Gap Balancing vs. Measured Resection Technique in Total Knee Arthroplasty

A goal of total knee arthroplasty is to obtain symmetric and balanced flexion and extension gaps. Controversy exists regarding the best surgical technique to utilize to obtain gap balance. Some favor the use of a measured resection technique in which bone landmarks, such as the transepicondylar, the anterior-posterior, or the posterior condylar axes are used to determine proper femoral componen...

Effects of Posteromedial Vertical Capsulotomy on the Medial Extension Gap in Cruciate-retaining Total Knee Arthroplasty

Introduction: Equality between the extension gap (EG) and the flexion gap (FG) is one of the most important factors for good clinical results in total knee arthroplasty (TKA). The gap situation during surgery has a wide variation. The EG needs to be widened in patients with varus knees that have a small EG. For this purpose, posterior soft tissue release is necessary to avoid additional resecti...

Periprosthetic bone remodelling in total knee arthroplasty.

ABSTRACT INTRODUCTION The clinical studies have shown that the displacement of the prosthesis components, especially of the tibial one is higher during the first year, after which it reaches an equilibrum position compatible with a good long term functioning. This displacement takes place due to bone remodelling close to the implant secondary to different loading concentrations over differen...

Flexion and extension gap balancing in revision total knee arthroplasty.

UNLABELLED Revision total knee arthroplasty presents a unique set of problems when attempting to balance flexion and extension gaps. Loss of soft tissue support and established deformity can make balancing difficult. One needs to balance the flexion and extension gap heights as well as medial and lateral symmetry, which may not always be attainable. We used a set of stepwise techniques to reest...

Is patella eversion during total knee arthroplasty crucial for gap adjustment and soft-tissue balancing?

INTRODUCTION Assessment of soft-tissue balance by the gap technique in Total Knee Arthroplasty (TKA) impacts femoral component rotation positioning. Proper femoral component rotation is a critical factor in TKA, both for adequate patellar tracking and in achieving a symmetrical flexion gap. Soft tissue balance assessment and gap measurements are performed at 90° flexion and with the patella eve...
